"Welcome to the Asia-Pacific NGO Forum Registration Website". You have three (3) options to register:
  • Option 1: Downloadable form in PDF format


    1. Click here to download PDF registration form and print.
    2. Fill out registration form.
    3. Submit filled-out/completed application form to the AP-NGO Forum 2009 Host Committee via email (ippo@mc.edu.ph) or fax (+632-433-8652). If submitting via email, a scanned copy of the filled-out
      registration form must be attached to the email.
    4. The AP-NGO Forum 2009 Host Committee will send confirmation of the receipt of your registration form.


  • Option 2: Downloadable form in MS Word format


    1. Click here to download MS Word format of the registration form and save the file in your computer.
    2. Open the saved registration form file and fill out.
    3. Submit filled-out/completed application form to the AP-NGO Forum 2009 Host Committee via email (ippo@mc.edu.ph) or fax (+632-433-8652).
    4. The AP-NGO Forum 2009 Host Committee will send confirmation of the receipt of your registration form.


  • Option 3: Online Registration


    1. For new users, click on 'create an account' button and fill out account information.
    2. After successfully creating a new account, log in using your email address and password.
    3. Fill-out the online registration form. Click 'save' button each time you encode information in every page.
      (Note: You can always go back to the information which you have not completed next time that you log in) 
    4. Click Submit button once you have completely filled out all items.
    5. The AP-NGO Forum 2009 Host Committee will send confirmation of the receipt of your registration form.
